Those machines that scan your boarding cards should tell you if it has double scanned a boarding pass. They should also tell you if you scan the card for the wrong flight. Both things have happened to me. Worse, they are supposed to check your passport when you board an international flight.

Agree with you @adam that turned around ANA flight seems awfully fishy. Plenty of safeguards to prevent exactly that situation from happening.
