Finicky hardware: It's old, relatively complex for the time, and in some ways badly designed, so plenty of different components can go bad. The original power supply, for instance, is a ticking time bomb that can fry your C64 if not monitored or replaced.
Incomplete without accessories: You can't just buy a C64 and then buy your cartridges and use it. Well, you could, but you'd be playing very few of the early, shitty games. You need a disk drive, a tape drive or a device that will emulate one of the two. The ideal is a disk drive, and a SD2IEC card reader, or an expensive Ultimate-II cartridge. You'll also absolutely need a joystick (which is also only 1 button); the keyboard is useless for gaming.
Region incompatibilities: Yes, it's a thing on the C64. It's not region locked per se, but there are PAL and NTSC versions, and some games (not all) do mind which one you use depending on how they're programmed. If you intend to buy games, you probably want whichever one is local to you, but for piracy PAL is preferable.
