Silencing of instances and users does not mean you can't follow them. It means they are filtered from the federated timeline. If you continue to see problematic content from users on an instance, you're following someone who is boosting or saying said content.
It goes without saying that it's recommended to unfollow them if it consistently causes distress.

One thing it does mean is that toots from silenced instances disappear invisibly from threads that the user opened, at least as it stands now. Just fyi in case you didn't know and it helps in deciding when to silence an instance or not.
