      It's the tool the Shinigami (they're not explicitly the be all and end all, gods of death, they're just supernatural beings in general, who are sort of implied to possibly be) use to extend their lives. They kill off humans with it, and get the remainder of their lifespan added to their own. Or something like that, it's been a while. Humans will die eventually anyways, they don't organize every single death, they can just kill people before that with the books. 
      
      There's different ones throughout the show and manga (since, every Shinigami has one, so they can be stolen and dropped into the human world)
      
      But the first one is one that's stolen by Ryuk (the main Shinigami guy) who...if I remember right, basically just drops it down into the human world for giggles.
      
      It's been a few years since I watched it, not really into anime or anything any more.
